If you just want to connect a trackball or a single spinner, and you dont need to use pure dos, go with the USB version. Although they both work perfectly well, I cannot seem to find any drivers for the serial version that will let me use all three mouse buttons. The USB version does not need any special drivers, and all three mouse buttons work automatically. However, if you want to connect two divice to the interface (one for each axis) such as two optical rotary joysticks, or two 360 degree steering wheels, or you need to use pure dos definately go with the serial version. It is much easier to connect two devices to the serial version, because the USB version only has one pin for ground and one pin for the +5v so the two devices will have to share those pins. On the serial version, if you plan on using two devices, everything comes ready to plug in and play.
