Unless something strange is going on, Windows doesn't use VESA or VGA modes (not with modern cards anyway), modern cards have there own Video Layer (.vxd) and Video Drivers (.dll), so when you go into Windows it won't know about the TSR because it isn't making any direct VGA/VESA calls, instead the VXD/DLL are making direct hardware calls to the videocard.
Only way to get Windows on an Arcade Monitor is by hooking into the VideoMode change request function and re-programming the clock speed that way, like what Saka does with is Windows interlace tools.
