Found one -- Saitek 8-button driver

Thanks, but I happened across a web page of someone who hacked the same type of gamepad. He used a Saitek 8 button driver. I downloaded and installed the driver. It works fine now.

With the "universal" driver I found earlier, buttons 1 & 2 wouldn't work if pressed simultaneously. I suspect that this was some kind of joystick driver because it had a hat switch in the calibration routine. Buttons 1 and 2 together were activating the left direction on the hat.
