By clicking you are effectively making it current and then your keystrokes are recognized by MAME32. Try shutting down all other applications, even ones on the System tray before launching MAME32 and see if it makes a difference.

It turns out that ATI launchpad was the culprit. I have an ATI all-in-wonder (which is great for memein' btw, tv out rocks) and it has a menu that works like the start menu that hangs out on the right side of the screen. This is the proggy that was giving me trouble. Thanks a lot
