I found that a joy and 8 buttons is a fairly good setup for Quakes, I can't remember the exact setup(it's macro 3 on my Power Ramp)
Here's a rough layout (not exact, but you'll get the idea):
Space A ALT Enter
Joy
Ctrl Z tab End
I think that's fairly close. I works OK for me, though your experince may vary :)
