I have an old electrohome 19" and I had to split the negative H-sync and V-sync. Twisting them together gave the same results you're getting. Here's a pic from Salty's site (I have the same monitor) Try running the two syncs separate and see if this helps. They are pin 13 (H-sync)and pin 14 (V-sync).

: OK just hooked my computer up to my Arcade monitor and after adjusting the Vhold and all the H and V adjustments the arcadeos looks ok (WOW that was HARD) now when I choose a game the games starts and the screen gets out of sync again.... How do I make it work right with mame and arcadeos???

: I hooked the red green blue and ground and H and V sync to the right wires.. ( the H and V hook to the ONE wire on the Monitor thats all it had)
