I've got almost the exact setup. I don't know which Japanese cab you have. I'm using Win98 booting to DOS, not actually Win98.

The Blade15 app is only for running Win98 for your aracade monitor in Windows. I'm not running MAME this way so I can't comment on that.

I use ArcadeOS, Blade3d, and VsyncMame. There is also a file for ArcadeOS on the VsyncMame site. It adds a couple other options to the ArcadeOS setup. Mostly resolutions, and sound options. I do have to select some specific res. for some games - mostly the verticals.

I have run Win98 on my cab with Blade15. Just to see if it would really work, and it does. I had it set at 640x480. Can't do better than that at standard res. I have a dual boot menu that I can select DOS or Win98. If I do nothing it will boot to DOS after a few seconds.

I'm using the I-PAC but not the USB. I think it is the same board, just different cables.

My cab already had speakers. I wired it through the JAMMA connector with the rest of the video/sound/controllers. I bought a set of speakers with an amp/sub woofer (for a PC). Then I hooked that to the PC, and the cab speakers to that. It gave me the amp and extra bass and it was pretty inexpensive.
