I've got the same problem and I'm using an onboard yamaha sound card plus it does the same thing on my other pc and it has an awe 64 card.
The audio problems for me started a long time ago when mame switched from wav to sam files (or was it the other way?)I first noticed problems with scramble and super cobra where the audio sounds real scratchy and kinda echoes but it used to be fine.
If anybody knows what fixes this let me know

: I have a similar problem with my Athlon 550 box and a SB 16. Are there known issues with MAME and a soundblaster 16? Doesn't happen with every game, but a goodly number.
