IPAC sticking problem

I'm using an IPAC (purchased about a month ago) in PS2 port mode (it supports USB but I'm not using it). Connecting it was a breeze... great job Andy! However, I'm seeing a problem that a joystick press occasionally does not register. Specifics:

1) I'm using the IPAC in test mode (coin A grounded) because the other two key sets conflict with various emulators I want to run.

2) The IPAC is connected via PS2 port to an Abit BH6 mobo and an old Northgate omni-key keyboard is connected to the IPAC pass through. Keyboard works fine (and is essential in my setup so I can't disconnect it). I have not tried it with the USB connection because I think you can only have 6 keys down at once then (right???).

3) Infrequently I will notice a joystick press not register. Seems to happen more often (or is just more noticeable) in robotron but I've had it happen in donkey kong and pacman as well.

4) Releasing the joystick and then pushing it in the needed direction again makes it register.

5) I don't think it is bad joystick switches (they are new and it seems more frequent when multiple switches are closed at once (e.g. robotron, joust, etc) than single joystick games (e.g. pacman).

My questions:

1) Has anyone else seen anything like this?

2) Anyone using the IPAC in USB mode? Can USB handle more than 6 simultaneous keypresses?
