Try setting your card to 640x480 AND reduce the refresh rate down to 60Hz AND ensure your TV or video cable is connected to your card. Reboot and see if it ungreys the selection.
I had exactly the same problem with TV-out on my TNT2 (ie: Greyed out selector), however doing the above fixed it. Still won't do simultaneous monitor and tv but will enable me to select one or the other.
