I have not. I use the L4 by PDPI. It is an ISA card that gives you 4 gameports for analog controls. Act Labs carries them at http://www.act-labs.com/pdpi_l4.htm
I have tried four 6-button stick pads with mine and it saw every single button press and stick movement perfectly. Mame (dos) does not support it (more accurately Allegro doesn't have support for it) but it is supported in Windows and works awesome with Mame32. It is nice to play 4 player Gauntlet like we used to.
