No, for the vgavsync part every vesa3 compatible cart will work.
You can use it if you have sync problems like tearing and little hickups and jerkyness. (For me this happens in a wind dosbox) For example the scrolling in Scramble or the movement of your ship in Galaxian. If you do not have these little but annoying issues you do not need the vgasync part of vsyncmame.
If you do have these issues. You can use it by changing a line in mame.cfg to vsynmethod=vgasync or use the -vgasync commandline.
Vsyncmame will then use vesa3 clock to sync more accurate with the game.
vsyncmame is just mame with these extra's. You can use arcadeos as a frontend for it.
