Thats the problem, you aren't using mixerset or setting the irq, dma on boot, its alright using diagnose to find what irq etc they are on but you need to lock them as well, games like quake, doom, and wolf do this for you mame does not, it tries to use what is already locked, what you need to do is put all your sb16 driver in c:\sb16 and edit autoexec.bat so it has this in it (change settings on blaster line to what where reflected when you ran diagnose.exe) :

SET SOUND=C:\SB16
SET BLASTER=A220 I5 D1 H5 P330 T6
C:\SB16\DIAGNOSE /S
C:\SB16\MIXERSET /P

and as if by magic.. sound works in mame :)
