: A card with TV out will usually come with a special cable to hook to a TV, the connector for it will usually be below the monitor connector.

: no problem, though you need to run one at a time, becuase an arcade monitor uses a different frequency than a computer monitor. If you leave the arcade monitor on in windows you'll damage it, if you're running ArcadeOS with your PC monitor on you'll damage it.

: Placing ArcadeOS in your startup folder might work... there is an older video card that had a "bug" that when you plugged in the TV-out it caused the wrong frequency to go to the monitor, the frequency it outputs is compatable with an arcade monitor.
