There was a V & H sync pin on my monitor (Kaga) also, but connecting up the V and H sync from the PCs VGA didn't work (picture still rolled every which way) - doing the "V & H sync twist" did.
Also, I had to change the V-sync parameter within the arcadeos menu structure to stop arcadeos from "rolling" even when the games were rock-solid.
Do your colours look ok? The colours on my monitor look a bit funny, but can be improved by cranking the gamma for dos Mame up to max (2.0). Hmmmm...
