I can't seem to find any version number for the Module driver but I checked out the installation CD and the following was in the readme file:

The driver that I'm using for my ELSA Gladiac card is 4.12.01.0206-0150.
I've had the module for about 6 months but haven't played with it until now.
I'm not sure about the shifting of pixels. In my tests (default settings), the image was shifted all the way to the right and the top portion was cut off. Unfortunately, I'm leaving tomorrow (outta State) for two months and don't have any time to figure out what's going on. Hope it works out!
