I'm running a DOS machine with an arcade monitor.
I'm using ArcadeOS it allows keys to be remapped in Real and Protected Mode. I have had a few problems with this but it works for a few of the games I was trying to play.

I did have a problem using one of the Sega Genesis Emulators. It allowed some remapping but there were a couple of keys that didn't work right. I got around it by using the new IPAC and reprogramming it at the start of Sega Emu. when it is done it returns it to the MAME keys. Adds a couple seconds at the start but it allows all keys to be remapped in the IPAC hardware so it is compatible with any software.

Don't know wht your setup is but this is how I got around it.
