I think I answered my own question from Oscar's web site. I believe that the issue with Optimouse assigning the secondary mouse to player 2 is the issue. Here is the solution I am going to try out later today:

"Three Mouse Devices

By using a DOS dual mouse driver, I have a PS/2 mouse and (2) serial OSCAR's
operating together on my test machine. It is accomplished by using DOS MAME
.37b11 or later and modifying your Autoexec.bat file this way:

-------------------------

C:\MOUSE\MOUSE.EXE DUAL

C:\OPTIDRV\OPTIDRV.COM /2

-------------------------

The dual mouse driver initializes the PS/2 mouse and the OSCAR connected to COM
1. These will both be assigned to "Player 1". The optidrv.com initializes the OSCAR
connected to COM 2, and it is assigned to "Player 2". Using this method, it is feasible
to have a PS/2 trackball (Imperial PS/2 arcade trackball comes to mind) and dual
spinners connected to a PC without using an additional interface! "
