There are several options, this one worked for me. I boot to DOS and run AOS Front End. I have 2 spinners I purchased from Fultra, A trackball from HAPPS and an OPTIPAC from Andy Warne.
The Trackball hooks up to port 1 on the Optipac, spinner 1 hooks up to the X axis on port 1 and spinner 2 hooks up to the X axis on port 2. the 2 serial ports on the Optipac plug into 2 serial ports on the back of my PC.
I downloaded Optimous.zip in order to have mame recognize the second spinner. I don't remember the exact settings, they are in the optimous documentation, but I had to force the mouse drivers to be microsoft.
