I don't know how old your monitor is, but I have a an old 19" CBO in same condition (power to the monitor and fuse is ok). My monitor is old, and has more than one fuse (make sure you check for other fuses on your board). There is a monitor trouble shooting PDF somewhere on the net. I've tried replacing the Horizontal and Vertical transitors, with no luck. I'm pretty sure now its just the flyback thats the problem. I don't know how to test for broken flybacks, but if I were you I'd start looking for tips on that.

: There is a white and black connector that goes from the isolation transformer to the monitor power in on the board. I measured the voltage on this connector with a digital multimeter. It measured about 128 or so volts (if this could potentially be a problem I can remeasure and give you an exact reading but I was under the impression that AC fluctuated and a 120v could read 118-128 or so....).
: I checked the fuse for continuity and it was not blown. The tube does not crackle (you know the sound that arcade monitors usually make when power goes to them) when the power is turned on the main power supply. The back of the tube (the little glass part right at the very tip end of the back of the monitor) does not light up like my other arcade machine.
