The SegaPC version of Virtual On - looked like a port of the Saturn version - had (I remember reading) an option for using two PC joysticks using a Y-splitter for the joystick port to simulate twinsticks. It may have also had a twinstick-like keyboard setup.

If you could find this, it might give a hint as to how the arcade version of VO and VOOT handled. The last copy I saw in a store was a couple years ago, though.

The PC version had online play too. It's probably worth picking up if you liked the arcade VO.

Something else I was thinking about was the feasibility of hacking GameShark codes for the DC version to remap the controls to be more twinstick compatible-like joker commands? Anyone good at GS hacking?

: I'm still curious to know if anyone who has real twinsticks, if you can side step slowly (one stick in neutral and one to the left of right). According to the gamepad setup, this isn't an option...just running/sidestepping quickly to the left or right.
