I put three buttons on my panel that have worked out great for me - A red one for ESC, Yellow for P, and Green for Enter. Hit green to start the game, red to exit back to the menu, and yellow to pause.
Under the panel I have three buttons tucked away for config, frame skip, and coin in. I haven't needed any adjustments that I can't do with these buttons.
