Depending on your TV system NTSC (60hz) or PAL (50hz) you will only be able to run Vsync on games that use either 50 or 60hz, otherwise MAME will come back with an error.
Most games are 60hz so you be okay if you are using NTSC.
Only way to run R-Type and other games smoothly is using something like VSyncMAME, AdvancedMAME or MonArc Plus.
