P2 button input problem w/I-Pac

Hi all! I've done a PC2Jamma conversion, and the controls (2 joys and 12 buttons) are hooked into an I-PAC.

My problem is that player 2 buttons 5 & 6 don't work consistently. They'll work when I've freshly rebooted the PC and they work when I run the MK.EXE keypress-test program included with the AdvanceMAME utilities, but when I actually play a game with MAME P2,B5 and P2,B6 become non-responsive after a few moments and stay that way until I next reboot the machine.
I've tested all the connections and re-wired the offending buttons directly to the I-Pac and they do work when I test them in DOS, but as I say, they die when I try using them in a game and stay dead.

I'm also having a problem with the "pass-thru" feature on my I-Pac.. in this case it just doesn't seem to be working at all, with any keyboard I try.

I thought I'd run these probs past you experienced guys here before I bite the bullet and mail Andy directly.
