I use advancemame for:
-filter options in old games to get that old blurry feel.
-hardware scanlines for the newer games.
-screensaver option
-blitrotate, every game gets rotated correctly on my vertial monitor.
-100% control over video modes on my pc monitor.
I have not tried vsyncmame in a while, so maybe some of the things mentioned are also supported by it...
