I've done a couple DC hacks (my second one was a DC arcade stick PCB after having all the same problems you are-) Did you cut off the wires to the analog pad? even if you aren't using it, that seems to be where the problems arises - at least it did for me. I don't know why, but w/o the analog pad connected, nothing worked.

: i´m still trying to get my dreamcast controlpanel to work(using two madcatz pads).I checked every connection and every solder point(looked and checked with my multimeter to diode testing,don´t know the other settings), so far everything was right. But no buttons worked.I´ve used one ground per pad and dropped solder onto the pgm button(to activate digital buttons c and z).
: So i decided to unsolder everything and just testing the pad pcb´s. The led on the first glowed green(as before), but the led on the second pad now glowed red and no buttons worked(on both pads).
