It usually means that it was a test revision of a game - or a test game that never was released. For example, Speed Coin is a prototype, and was never actually released in arcades (except possibly as a test). Others, like the Trog prototype, are just earlier revisions, like a Work-In-Progress, that were somehow acquired and dumped...

In the case of Trog, it's almost like playing a whole other game. My guess is that they tested a few locations with the prototype rom set, it didn't do too well, and they went back and changed some things and rereleased it...
