I've always been partial to the NeoGeo button layout, personally, so I'm trying a combination NeoGeo/SF2 layout for my SNES pad. (See the link below.)
When I first worked on setting up the buttons, I set my hand on a piece of paper and drew a half-circle around each of my fingers. Then I made an X at the center of that arc and used a compass to draw a button-sized circle around that point. Also, I was reading Lanzer's guide, and he said he'd placed his buttons 4 cm apart at the centers. I re-arranged my buttons so that they had the same basic layout, but were all 4 cm apart, giving the layout a nice even look.
Adobe Illustrator is good for setting up layouts. Just make as many buttons as you need and scoot them around 'til you find something you like.
