You are basically right. A mouse is a small trackball repackaged for function.
The middle scroll wheel would probably not be userful too you. Mame supports X & Y analog inputs. The scroll wheel is adding another input which MAME will not see. Sorry.

From looking the box, it seems to be a mouse with a tiny trackball that is used for scrolling both vertically AND horizontally. (correct me if I'm wrong)
