Here is what I use:
Right: Right Down
Left : Left Up
Up : Up Right
Down : Down Left
make sure it has both directions listed like it is listed above (doesn't matter which comes first)

: : Qbert can easily be played with an 8-way joystick. Just configure Mame to only accept the diagonals for the movement directions.
