IPAC problem - help requested from Andy or anybody else who knows the answer

I have what I believe to be the first revision of the IPAC. After finally getting it installed I have been very happy with the installation.

However, a weird problem has popped up which I cannot resolve. My cabinet is set up to support 2 people playing simultaneously and when I choose games like SmashTV or 1941 which allow either player to continue by inserting additional coins, the Player 1 (the "1" key) continue button stops functioning.

I have also observed that if player 2 is prompted to insert more coins during the course of the game, Player 1 is then able to continue on when the button is pressed.

This has occurred on an earlier version of MAME as well as the latest 0.37beta10 release. Any help you can offer would be appreciated.
