MAME is an application that can either be run under Windows in a DOS window OR in real mode DOS. Running in real mode is best for stability and performance.

MAME32 is a NATIVE Win32 application. This means that you must run it in Windows. The downfall to that is you get slower performance and MAME32 is usually a few versions behind MAME (it takes them a while to port it)
