Visible area confusion with AdvanceMAME and arcade monitor

I'm using ArcadeOS 2.41 with AdvanceMAME.37b9 and an ATI video card on a 25" 15.75KHz arcade monitor. I've run the thingy in AdvanceMAME to determine all the resolutions and scan rates that work on my monitor. Now when I launch a vertical game like Pac-man, it looks like it's going to fit perfectly... the bounding box around the info screen that shows the screen size fits on the screen perfectly... but when it gets to the actual game there's a huge bunch cut off the top and bottom. I don't get it. If I look at the game info, it shows that it's only 224 pixels tall, and the video mode that AdvanceMAME is selecting says 320x242 or something like that, which is more vertical pixels than the game needs. What's the deal?
