Problem connecting from VGA to Arcade monitor!

Hi,

I just finished my VGA 15pins to Arcade monitor connector. It displays fine now with 640x480 16 bit but it displays multi set of screens side by side. As you can see from the Starwars screen that I included here, there are two set of screens. What am I doing wrong here?

Believe me, I tried all the adjustment from the HV hold, center and position knobs but no luck. Luckly all the knobs are in front of the monitor so I didn't have to break my back to adjust from the back. As
you can see from the Starwars screen, the knobs are in very easy to reach location.

There must be a problem with my VGA to Arcade monitor cable but I can't find anything odd. I followed step by step creating this cable and checked for the continuity for each pins twice to make sure.

My medium resolution 25 inch Nanao monitor has only 5 lines going to tub pcb so I combined pin 13 and 14 to Tube pcb HV Sync.

Is there anything I should be aware of besides just making the VGA to Arcade Tube adapter?

Maybe I need a different VGA card?
