Just noticed this thread. Seems that everything is working OK now but just to mention two things:
If the I-PAC is going in to test mode this must mean one of the buttons is stuck or shorted. In later versions only the coin 1 input can cause it to go into test mode.
As a general fault-finding method the best thing to do is go into DOS or Notepad. The I-PAC should behave exactly like a keyboard and send the characters as shown on the code chart. Note that some inputs will not generate a character in MAME mode, as they send "alt", "ctrl" etc. This is the reason for test mode, in which all inputs send a character. I have only had one actually fail in service, and that was pinpointed down to a faulty ground connection that applied 240 volts AC to the inputs! (Hi Tobias!)

: I'm having problems with an I-PAc I've installed in my cabinet. Over the last few months the 1 player start input has been giving my problems, sometimes staying on and then sometimes I'm not able to get it to respond at all. Now it seems to be stuck on because when I start my PC, the I-PAC goes into test mode. I've tried to remove the MAME jumper but that didnt help. Anyone got any ideas?
