1 common ground wire to each button and joystick and then another wire for each input device eg. Button 1 to Button 1 input on the IPAC.

I think one of the inputs are used to join them together make only 55?

IPAC uses it own shift functions.

I have not, but there is no reason why not you could put it in a small plastic box and then screw that to your arcade machine.
