Using an extra 'fire' button to enable a 'config-mode' sounds like a good idea, since I think a lot of people would install at least 4 buttons on their CP's for Neo-Geo games, and maybe 6 for SF2, etc.
Using the 1P start is also good. That won't interfere with anything before starting a game.
The only problem I could see is not knowing what setting you picked. I guess if you chose the wrong setting, you could change it easily, but it might be nice to have an indicator for what setting you picked.
A three position switch would help tell you what setting you picked, although it might clutter up the CP. It would also limit you to 3 settings.
My VCR remote control can control different brand TV's - but you need to configure it for your brand by pressing "Power ON" + some other button (like Fast Forward etc.).
I always forget what the right button combination is and end up pressing every button until the TV turns on.
I could see myself forgetting what the 'sensitivity config button' was.
What about three LEDs connected to the sensitivity settings?
That might look cool - but that would also limit you to 3 settings.

What about recompiling MAME so that all the rotary joystick games accept the same setting?
Would that work?
