Actually, what you really want is to map to the diagonals and not to the primary directions. Let's say the keys for your joystick are mapped to UP = U, Right =R, Left =L, and Down =D. (For an analog stick, just move the stick in the desired direction.)
Also, Qbert is setup odd in MAME in that the game moves diagonally and the MAME interface uses UP, DOWN, LEFT, RIGHT. Play the game with the keyboard to know which key to map.

Let's assume UP in MAME is Diagonal UP LEFT. To map to the diagonals, enter the setup for UP, press U and then imediately press L. Mame will read UL for the key combination.

To map only to the diagonals, press U and then immediate press L, wait a second and press U quickly twice, wait a second press U and then immediately press L, wait a second and press L quickly twice.

At this point, MAME will read UL not U or UL not L. Repeat this for the other three directions.
